Terms & Conditions for Cacao Ceremonies, Sound Healing & Reiki

This agreement is entered into between Johanna Olivas, Luna Serenity, operating at various locations in United States and worldwide, and yourself ("I") for this and future visits.

 

DISCLOSURE ABOUT CACAO: Sacred ceremonial cacao is prepared in such a way that, the properties of it alone increases blood flow to the brain by 30-40%, and the actual oxygen in the blood is also increased by 20%. Along with this and the meditations and healing offered in this ceremony, you can greatly expand the energy of the heart center for deeper awareness, healing, joy and creativity. We ask that if you have high blood pressure, heart issues, allergic to chocolate, are pregnant, or are prone to severe anxiety or are on antidepressant or any other medication, that you do not consume the full amount of cacao served in these ceremonies. We require you to let us know in advance of a ceremony.

 

DISCLOSURES ABOUT SOUND HEALING: Johanna Olivas, is not a licensed medical care provider and is not rendering personal medical advice or treatment, and has no expertise in advising on, diagnosing, examining, or treating medical conditions of any kind, or in determining the effect of sound therapy on any medical condition. You acknowledge and agree that when participating in any class or workshop, and/or when using any products or services, there may be the possibility of detoxification symptoms or physical injury, and you assume the risk and responsibility for any such results.

DISCLOSURE ABOUT BREATHWORK:

I have voluntarily enrolled in this Breathwork activity. I understand that I am under no obligation of any kind to participate in this Breathwork activity and I voluntarily enter this into this Waiver and Release of Liability.

 

I understand the Breathwork is a personal growth experience designed to enhance the quality of life, and is not a substitute for psychotherapy and does not substitute for therapy if needed, and does not prevent, cure or treat any mental disorder or medical disease. I understand that I am responsible for creating and implementing my own physical, mental and emotional wellbeing, decisions, choices, actions, and results. As such, I agree that the Breathwork facilitator(s) is not and will not be liable for any actions or inaction, or for any direct or indirect result of services provided by the Facilitator(s).  I understand that this Breathwork activity is not medically supervised and that Johanna Olivas is neither licensed psychotherapists nor licensed medical professional and that breathwork is not a substitute for any medical diagnosis or medical treatment.

 

I understand that this Breathwork activity will involve strong connected breathing and may include guided meditation. I understand that Breathwork can involve dramatic experiences accompanied by strong emotional and physical responses or releases.

 

I understand that I might find Breathwork physically, emotionally, and/or mentally stressful. I hereby affirm that I am in good health and able to participate in this activity. I do not have any physical or mental conditions which would impair my ability to engage in this activity or which would otherwise endanger my health during this Breathwork activity, or which would cause any risk of harm to myself or other participants.

DISCLOSURE ABOUT REIKI: 

I understand that Reiki is a gentle, hands-on energy technique  used for stress reduction and relaxation. I understand that Reiki practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. I understand that Reiki does not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological aliment I may have.

 

I understand that Reiki can complement any medical or psychological care I may be receiving. I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial. I acknowledge that long term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself.

 

I assume sole responsibility for my own health and for the results of any sessions provided by Johanna Olivas that may affect my health in any way. Treatment/s will not replace conventional medical diagnosis or treatment.  I will continue taking medication prescribed by a licensed medical physician and will continue to follow his/her instructions. I release Johanna Olivas from all legal liability during my participation in the Reiki treatment/s, as well as the locations where Johanna Olivas offers Reiki. All information received by me from  Johanna Olivas is accepted with full knowledge that any action taken by me as a result of the information received is my complete responsibility.

 

ASSUMPTION OF RISK: I agree that I am voluntarily participating in a Cacao Ceremony, Sound Healing or Reiki session at my own risk and assume all risk of injury, illness, damage, or loss to me or my property that might result, including, without limitation, any loss or theft of any personal property, whether arising out of the negligence of Johanna Olivas or otherwise.

 

RELEASE: I agree that on behalf of myself (and all my personal representatives, heirs, executors, administrators, agents, and assigns) to release and discharge Johanna Olivas (and its affiliates, related entities, employees, agents, representatives, successors, and assigns) from any and all claims or causes of action (know or unknown) arising out of negligence from any of its staff, whether active or passive. 

 

This waiver and release of liability includes, without limitation, injuries which may occur as a result of a) any equipment or facilities which may malfunction or break, b) improper maintenance of any equipment, premises or facilities, c) negligent instruction or supervision, d) slipping or tripping and falling while on any portion of a premises or while traveling to or from any of the activities, including injuries resulting from staff or anyone else's negligent inspection or maintenance of the facility or premises.

 

ACKNOWLEDGEMENTS: This release is not intended as an attempted release of claims of gross negligence or intentional acts. I acknowledge that I have carefully read this waiver and release and fully understand that it is a release of liability, express assumption of risk and indemnity agreement. I am aware and agree that by executing this waiver and release, I am giving up my right to bring a legal action or assert a claim against Johanna Olivas’ negligence. I have read and voluntarily signed the waiver and release and further agree that no oral representations, statements, or inducement apart from the foregoing written agreement have been made.
